WELCOMEto Lakeside High School Health Office webpage! Our office is staffed with a Health Technician, April [email protected]/ Health Office 253-7318), as well as a credentialed school nurse, Lily Carter, RN [email protected] 285-4077).
 
The main goal of the health office is to provide basic first aid and health care assistance to students as needed. Then, students are either returned to class or sent home.
 
 
***Note: It is very important to update all contact phone numbers as so that we can reach you. Also, please provide current phone numbers of local individuals (if parents are not available) who have your parental permission to pick-up their student (s) ill or injured.
 
Immunizations: Each student, upon admission, will need to present a provider-documented Immunization Record with the required immunizations according to the California State Department of Health and Riverside County Health Department.
 
These records need to include:
1. The student’s name and date of birth
2. Type and date of each immunization dose
3. Name of clinic or physician administering dose (must be signed or stamped by clinic).
 
Health Conditions/Concerns: At the beginning of each school year, parents complete an Emergency Contact form (via Online Registration) that has a place for parents to include any health concern regarding their student. This is extremely important information to have, especially in the event of an emergency. Please note any medications that student take at home and at school. If you need to discuss any concerns regarding your student’s medical condition and medications, please contact the school nurse in person, phone or email above.
