ATHLETIC CLEARANCE
The Athletic Clearance is the first step to becoming a student-athlete at Lakeside High School. The process must be completed every school year and includes a physical from your doctor. Everything is online, if you have not done so already, create a login at HomeCampus.com, if you already have an account be sure you register for the next school year. If you have any questions, feel free to contact [email protected] or [email protected] for help.
First step: Create an account on www.homecampus.com/login
(We suggest using your students email address so we can assist them if they need help at school)
Second Step: Upload the completed Physical Page and Signature Page onto Home Campus. The physical will require a stamp, doctors' signature, and date the physical was done. A physical will last one year and will need to be redone every year.
